Lexi Johnson, LMSW

Lexi joins West with a strong background in hospital-based social work, patient advocacy, and care coordination.

Most recently, Lexi served as a Social Work Case Manager at Ascension Saint Thomas Hospital West in Nashville, where she collaborated with interdisciplinary healthcare teams to develop individualized care plans, address psychosocial and financial barriers, and connect patients and families with supportive services.

Her previous experience also includes working with children and families in foster care, providing trauma-informed support, and coordinating resources for individuals facing complex circumstances.

Lexi earned her Master of Social Work from the University of Alabama. She also holds a Bachelor of Social Work from Lipscomb University and is a Licensed Master Social Worker in Tennessee.
