1. Specialty Pharmacy Pharmacist (Full Time)

This position provides accurate and prompt drug therapy as well as current drug information and drug utilization reviews.  Work is performed under the general direction of the Director of Pharmacy Operations.

Responsibilities:

  • Responsible and accountable for the provision of safe, effective, and prompt medication therapy
  • Manage daily operations and ensure delivery of patient services according to company standards and expectations
  • Advocate clinical activities such as medication therapy management and adherence programs
  • Drug information services for patients, caregivers, and providers on drug therapy, dosing, interactions, stability, new drugs and other treatment modalities.
  • Encourage teamwork through cooperative interactions with pharmacy staff and other clinical staff
  • Provides discharge medication review, reconciliation, and counseling as appropriate
  • Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
  • Evaluate and verify the physician’s plan of care, treatment plan, drugs, doses, and labs for therapeutic appropriateness
  • Restocks and audits drug inventory as required
  • Performs other duties as assigned.

Requirements:

  • Bachelor’s Degree in Pharmacy AND completion of Oncology Pharmacy curriculum within 1 year of hire, if not BCOP
  • 1-2 years related experience OR 5-7 years related experience and/or training or equivalent combination of education and experience
  • Licensed or eligible for licensure as a pharmacist in TN, MS, AR

2. Clinical Integration Pharmacist (Part Time)

This position provides accurate and prompt drug therapy, as well as current drug information and drug utilization reviews. Work is performed under the general direction of the Director of Pharmacy Services.

Responsibilities:

  • Manage daily operations and ensure delivery of patient services according to company standards and expectations
  • Works with multiple departments and sources to accommodate the needs of our patients; this includes claim processing, customer service, and serving as liaison between patients and other members of the patient care team
  • Advocate clinical activities such as medication therapy management and adherence programs with a focus on quality improvement
  • Drug information services for patients, caregivers, and providers on drug therapy, dosing, interactions, stability, new drugs, and other treatment modalities.
  • Encourage teamwork through cooperative interactions with pharmacy staff and other clinical staff
  • Assists in the coordination of care for patients enrolled in EOM and/or other West Clinic clinical integration programs
  • Leads the smoking cessation program for West Clinic patients
  • Provides discharge medication review, reconciliation, and counseling as appropriate
  • Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
  • Provide oral medication education for all new start West Cancer Center patients
  • Serves as the prior authorization pharmacist lead, when required
  • Serves as point of contact for outside specialty pharmacies
  • Serves as the liaison between the patient and provider for oral drug adherence and side effect management
  • Serves as backup for pharmacists in infusion and oral service lines as needed and perform additional duties as determined by management
  • Effectively participates and contributes to staff meetings, committees, and departmental work groups to share best practices, improve processes, and/or other items relating to patient/provider engagement
  • Performs other duties as assigned.

Requirements:

  • Bachelor’s degree AND completion of Oncology Pharmacy curriculum within 30 days of hire, if not BCOP
  • 1-2 years related experience OR 5-7 years of related experience and/or training or equivalent combination of education and experience
  • Licensed or eligible for licensure as a pharmacist in TN
